Expert is a fellowship trained urogynecologist who specializes in the treatment of urinary incontinence, pelvic prolapse, fecal incontinence and related disorders of the female genito-urinary tract. He has been the Director of Urogynecology at Mount Auburn Hospital, a community teaching hospital of Harvard Medical School, since 1995, and Director of Urogynecology at Beth Israel Deaconness Medical Center, since 1999. He has been active in clinical research involving incontinence and prolapse, frequently lectures regionally and nationally, and is an ad hoc reviewer for a number of peer reviewed journals in urogynecology, pelvic surgery, and obstetrics and gynecology. Expert is a consultant for many medical device companies in the field of urogynecology, and holds a number of patents in the field. Several of his inventions have been commercialized, including a sling for urinary incontinence and a personal device for behavioral modification for overactive bladder. His TOPAS sling for fecal incontinence is currently undergoing clinical testing in a multicenter prospective trial.
Expert is a recognized authority in minimally-invasive gynecologic surgery, including laparoscopic surgery. Although he lectures and teaches on all aspects of laparoscopic surgery, his major fields of interest are laparoscopic reconstructive surgery for pelvic organ prolapse, and supracervical hysterectomy. He is well known for his work in laparoscopic sacrocolpopexy and sacrocervicopexy for uterine prolapse, and recently published a series on this latter procedure. He pioneered a novel approach to laparoscopic supracervical hysterectomy (LSH) with his colleague, Dr. DiSciullo, which involves transcervical morcellation of the uterine fundus, thus avoiding enlarging any of the 5 mm abdominal incisions. A retrospective study of their experience has been submitted for publication. Expert is a consultant for a number of medical device companies that bring laparoscopic and other gynecologic surgical devices to the market. He is a national preceptor for a number of companies, including Bard, Boston Scientific, Gyrus / ACMI / Olympus, and Ethicon Women's Health & Urology.
As a urogynecologist. Expert has been involved in the evaluation and management of fecal incontinence. Surgical treatment in the past centered on anal sphincteroplasty, and the long-term published results were disappointing. Expert is the inventor of the TOPAS (Transobturator Post-Anal Sling), a novel implantable polypropylene mesh sling that restores the function of the puborectalis and recreates the normal anorectal angle in the continent woman. A recent prospective, non-randomized multicenter, pilot study of TOPAS was completed, and a multicenter, pivotal trial is soon to be underway.
