Expert has significant experience in all aspects of microsurgery. He has an established clinical practice in which microsurgical techniques such as nerve reconstruction, vascularized bone transplantation, replantation, toe transplantation for hand reconstruction, and soft tissue transplantation are routinely utilized for limb salvage and restoration of function. He is an associate editor of the journals MIcrosurgery, and Annals of Plastic Surgery.
Expert has extensive experience in microsurgical breast reconstruction. He has expertise in transplanting a number of flaps including the TRAM, DIEP, TUG, Latissimus, SIEA, SGAP, and IGAP flaps, and designs the reconstruction based on a particular patient's body habitus. His philosophy is that no one flap is universally the best flap for every patient and the best overall result is usually obtained when the risks and benefits of a particular procedure are optimized. An example is use of excessive abdominal tissue resulting in an abdomino-plasty and aesthetic recontouring at the donor site while providing excellent quality and quantity of tissue for symmetric breast reconstruction at the recipient site. The same would be true for use of thigh tissue when the patient is less satisfied with her thighs or has no abdominal wall tissue to spare.
Expert has extensive experience with the development and application of microsurgical strategies for optimization of hand form and function after traumatic, post oncologic resection, or congenital deficit. Microsurgical techniques such as replantation, toe transplantation, nerve reconstruction, bone transplantation, and soft tissue transplantation are combined with other synergistic techniques such as metacarpal and interphalangeal joint replacement, bone transport or distraction osteogenesis, distraction histiogenesis, and tendon transfer. Because of his creativity in dealing with challenging injuries, Expert has been invited to participate in panels on innovation in hand surgery at the American Society of Plastic Surgeons (ASPS).
Expert routinely deals with difficult reconstructive cases in which there is bone injury or deficiency, whether isolated or in conjunction with other tissue loss. Optimal success in these cases requires a number of approaches to healing or reconstructing bone. These include difficult fracture management, and techniques to replace bone such as bone grafting, vascularized bone grafting, and bone lengthening or distraction osteogenesis.
Expert training in plastic surgery and subsequenty in microsurgery. The combination of these disciplines provides an appreciation of the anatomy of the entire body, an understanding of the aesthetics of the body, the function of the entire body, and the tools required to successfully restore form and function.
Expert routinely deals with limbs threatened after trauma, post oncologic resection, or chronic diseases such as peripheral vascular disease, diabetes, and neuropathy. Microsurgical techniques such as replantation, soft tissue transplantation, vascularized bone transplantation, functional muscle transplantation are routinely utilized to restore the limb, optimize length for prosthesis application, etc. Expert works closely with orthopedic surgeons versed in Ilizarov application and bone transport for complex upper and lower extremity salvage. Nerve reconstruction, tendon transfer, restoration of functional motor units are utilized to optimize function.
